Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
The Dental First team believes that everyone should be able to have access to dental care regardless of their financial situation. We try to give you many treatment plan options tailored to your specific needs and prioritize treatment as needed. Every person is different, so the cost of treatment will vary from patient to patient. We will discuss the fees and help you choose a payment option before you begin treatment so that you know exactly what to expect without any surprises.

- In-House Dental Membership Plan called “Dent-Assure” (We try to offer our patients without any insurance benefits a reasonable way to be able to afford their dental care.)

Dental Insurance
We are in network with most major insurances and will submit claims on behalf of the patient. We also offer an in-house discount insurance plan called Dent-Assure. Please contact our office for insurance details. We look forward to hearing from you.
We try to obtain all of you dental benefits from your insurance company before your first visit with us and will gladly file your dental insurance claims for you. Since we have a lot of experience with insurance companies we will do all we can to maximize your benefits. We accept both in-network and out-of- network plans. Although we are Preferred Providers for most major insurance companies, including:

CareCredit
CareCredit has worked very well for our patients to help them receive needed treatments. It’s a great option to help break up the cost of your treatment into low monthly payments with several different term lengths. CareCredit will pay the cost of your treatment so you as the patient do not have to pay anything out of pocket up front. They will setup a no interest payment plan for you if you pay in full by the end of the promotional period as Dental First pays your interest!
